Create and run annual (?) OTel contributor survey #27

Related to open-telemetry/community#370, I'd like us to create a survey for all OTel contributors. I have been looking at some prior art in the k8s world (e.g., https://github.com/kubernetes/community/issues/3969 and https://github.com/kubernetes/community/blob/master/sig-contributor-experience/surveys/2019/contribex-survey-2019.md) and will get started by:

I'd like to close this issue by Sept 18 which should give plenty of time for feedback.

Re the original message here: this issue will not be closed by Sept 18 (today) since it's been coupled to the annual election process in order to minimize the number of surveys/polls we need to nag about. :)

I wonder if we want to entertain an idea of requiring filling up the survey for voting eligibility?

@SergeyKanzhelev I was just thinking that we somehow tie it to the ballot-submission process itself (method TBD)... it should only take a minute.

Last time we were using CIVS web site. Same as k8s using. It only allows voting without additional questions submission. Other tools might be fine I guess. If we want some contributors survey combined with the voting, we can entertain this idea here: https://github.com/kubernetes/community/issues/5096 - k8s will be building a new voting experience this year.

opinion, here, not strongly held: I believe that survey can cover bigger population than just eligible voters list. Eligible voters are quite engaged already. Also survey can be complete without the need to give us an e-mail (we need e-mail to guarantee a single ballot per eligible voter). So not sure it is the best idea to combine specifically this survey with the voting.
