Some tokens are initially capitalized because they are at the beginning of a sentence or set in title case. Some termcheck patterns use case="keep".
We could add some sort of flag to the above classes of initially capitalized tokens. This flag could then be used to determine that termcheck needs to check the word both with and without initial capitalization.
