Ignore citetitle elements when formatting xrefs #400

Currently, the entities for SLE book names contain the citetitle element. These entities are then used for the title element itself (where we already ignore global emphases and citetitles, I think but this needs to be checked), and then in turn are used for formatting the xref content.

We should make sure to ignore the citetitle tag when formatting xrefs and intra-xrefs as well.

So basically, it would be great if references are formatted correctly and not like in the screenshot below.

I think, we talk about different things. In the screenshot, we have two cross-reference types:

The first are rendered as link, is clickable, and underlined. The intra-xrefs are resolved, but are NOT clickable.

Could you elaborate @dmpop?
