Concatenate Bootstrap and bootstrap plugins CSS files from 4 to 1 fil… #73

I have created a lot of branches (over 2 dozen apparently) that I think have improvements but I'm not sure how to consolidate them (and some are probably too customized to be of interest) so I'll create pull requests for the branches that I think have the widest appeal.
