Here's the legislation

Section 24, 2 (a) refers to the 'form' not entirely sure if it means a proforma - specifically their form, or if form there is a synonym for structure

Oh no! Please say more about this? What do you mean by 'test that QLD will accept an email RTI request'? Did you send a request by email and they rejected the request?

I see one example (Department of the Premier and Cabinet) where the guidance on making an RTI or PI request includes email https://www.premiers.qld.gov.au/right-to-info/make-application.aspx

Here's some relevant information we got from the QLD information commission:

The RTI Act and IP Act as they currently stand both require applications to be made on the approved form, which is available as a downloadable PDF and an online form, the latter of which can only be used for applications to departments and Ministers. These are both available on the https://www.rti.qld.gov.au/ website. Agencies and Minister do have a discretion to accept applications which are not on the approved form, but only if the application includes all of the information required by the form and doing so is not required.
