Alert emails duplicate updates #614

When subscribed to two separate entities which cover the same request, alert emails duplicate details for any given update.

For example if following a specific request and the user who posted that request, any update to the request will be in the email twice under separate headings.

New updates for the request 'Server log files'

Australian Bureau of Statistics sent a response to Jackson Gothe-Snape (September 02, 2016) 
"Dear Mr Gothe-Snape Please find attached the notification of charges estimate for your Freedom of Information (FOI) request.   (See attached file:..."

observant1 added an annotation (September 02, 2016) 
"Evidently the ABS doesn't employ any linux administrators, a simple terminal command should export this data to a text file in around 30 seconds."
