Sometimes system crash occurs when clicked "corrected record" in record audit page #6179

Bug description: This is a very corner case. Sometimes system crash occurs when clicked "corrected record" in record audit page Steps to reproduce:

  1. Login to National registrar
  2. Advance search a birth declaration with status registered
  3. Observe the corrected record action of any record

Actual result: Sometimes system crash occurs when clicked "corrected record" in record audit page

Expected result: should be able to view modal of corrected record

@SyedaAfrida have you ever seen this happen in Farajaland? I'm trying to understand if it's a core bug or a configuration error in Niue config 🤔

@rikukissa I didn't found this bug in Farajaland. Found this in Niue environment

I've seen something similar happen in Cameroon and it was something to do with a configuration. @Zangetsu101 when we say this it was something to do with mismatching field names in the form configuration. I don't have enough to research the exact cause of this now but maybe we could add form config validation to prevent this from happening
