The issue was that the apache config was using obsolete syntax. It has now been updated. That fixed all but one of the asset issues.
The remaining issue was getting requirejs. FSR, the production version of requirejs isn't included in the build. I'm not currently sure what the issue is, but as the dev version was included that is used instead.
Clicking around in the appliance found some other issues which have also been fixed in this branch:
The vagrant virtual machines had no swap and not enough memory. If MIA is being built on something other than one of the vagrant machines in this repo, it is left to however provisions that machine to ensure sufficient memory and swap.
Fixed an issue where mia and modules would not start following a reboot.
