Final document annex formatting issue #219

it seems that there is a formatting problem with the order of annexes in the final document : annexes A and B (that must be B and C) are listed as subsections of annex C (that must be A)

you have the wrong header-level in some annex files (annex B and C). Please make sure your first heading is level 2: ==, not level 3: ===
