Sweep UML for "outfall" term. #226

I just noticed "HY_RealisedOutfallType" still in the description of HY_HydroLocationType, we need to do a thorough sweep of the UML model to find issues like this.

I am planning to go through the descriptions of packages, classes, properties, etc. in the UML model as soon as the text of spec doc is final, latest after posting the draft spec in pending docs.
