Assorted set of formatting changes #35

No content change, just formatting fixes & improvements

Annex B should be a level 1 header. Why do you want to demote it to level 2?

If you look at other annexes (such as https://raw.githubusercontent.com/opengeospatial/geotiff/master/GeoTIFF_Standard/standard/annex-history.adoc), you'll see they use level 2. I did that change since otherwise asciidoc emitted a warning

I guess the reason for level 2 is that level 1 is taken by the = {title} annotation in https://raw.githubusercontent.com/opengeospatial/geotiff/master/GeoTIFF_Standard/standard/GeoTiff.adoc

@rouault Good point about the paragraph numbering. I think what we want is numbering like we have in Annex A. I'll see if I can make this happen.

Changes were merged through other pull requests. This was necessary to resolve the conflicts.
