cover page to include editors, submitters, contributors #24

related to Issue #22

  1. Update template with entries for editors, submitters, contributors.
  2. Change cover page to show each.
  3. Should affiliation be included? We do in the body of the text, so no reason not to put on cover, plus gives organizations visibility.
