Offer mini-survey after upvoting / downvoting / commenting #237

I see this as a corner curling over that when clicked would offer the user a mini-survey that asks them if the annotation they're taking an action on is relevant to the conversation. I think we'll want to keep it to 3 questions, and they could be:

  1. How relevant is this to the conversation?
  2. How easy is this easy to understand?
  3. (not sure)

If this is specific to annotating, maybe:

  1. Is this relevant to the text you just highlighted?
  2. Is this a constructive comment?/Does this add to the conversation?
  3. Have you made this easy for others to understand?
