migrate the old symptom_fk field to symptoms for elcid.PresentingComplaint and walkin.Symptom #1604

for the models walkin.Symptom and elcid.PresentingComplaint. We have an fk_or_ft field that we no longer use. Instead we use the symptoms field.

So that this does not confuse users in the search we would like to hide this field.

Please migrate all fk values to symptoms.
