Start/stop dates of antibiotics #253

On the antimicrobials field, although there is a space to enter the start and stop date and who will administer each antibiotic, i dont think that this will be sufficient because often the same antibiotic will be administered by more than one service on more than one occasion.

For example, a patient has been admitted to hospital and has been referred to OPAT, they have been assessed by the opat team who have added this information to elcid: Ertapenem 1g OD Started: 10/1/14
Stop: 30/1/14 Delivered by: Inpatient

The patient is currently having this administered as an inpatient but is then discharged home on 15/1/14, so it is now been given by the district nursing team.
So the date of the first dose of this being given by the district nursing team would be 15/1/14. This date is crucial because this is the date from which i start counting the number of 'bed days' saved. I will stop counting on the date that they have the last dose given by the district nurses.

Sometimes the patient might have the same antibiotic given for 2 days as an inpatient, then come to clinic for the first 3 days to have antibiotics administered but then have a district nurse to administer the antibiotic thereafter.

Capturing the number of days that the patient had the antibiotic given by: Clinic, Community Nurse, Self Admin etc is really important to audit.

I think that the 'administered by' field will need a look up list to allow this: Inpatient administration Clinic administration Community Nurse administration Self administration Carer administration Other.... (free text just in case)

A solution could be to add subfields of 'administered by': Inpatient form ../../.. to ../../.. 'administered by': clinic from../../.. to ../../.. 'administered by': community nurse from ../../.. to ../../.. etc. I suppose these could have an 'add' button so that you can add multiple 'administered by' fields.

Also there is no way of differentiating between this being a recommended antibiotic or an antibiotic that is actually being administered. Am i right in thinking that we agreed that this field would only contain antibiotics that are actually given and proposed antibiotics would be written in the outstanding issues or diagnosis/issues field?

pt 1:

We need a quick way to duplicate an antibiotic, changing only the person delivering, and also changing the end date of the open antibiotic to today - two people can't be delivering the same antibiotic at the same time.

We would like to collapse these antibiotics in the list view
