capturing reason for change in antimicrobial therapy #261

One time point it would be good to capture some data is when patients change antimicrobial therapy

A possible point at which to capture this information could be when clicking the 'stop antibiotics' field in the 'antimicrobials' column

The questions to ask here would be...

1: Why were antibiotics changed? • planned stop of iv and switch to oral • planned stop of all abx • unplanned stop: if yes, then need to specify why e.g. 1: line issue (details should have been recorded in 'lines section') 2: abx intolerance / allergy issue (details should have been recorded in 'lines section') 3: logistical issue (free text detail e.g. 'district nurses unable to administer') 4: disease progression 5: antimicrobial resistance 6: patient preference 7: other (free text)
