Suggest we divide symptom duration similar to #359

I think we should do <3 d <10d 10-21d 21d-3 months

3 month This fits better with the way we group disease by incubation period (and also fits with the BIS fever returning traveller guidelines which most of the SHOs use.

I think we have multiple entries as multiple entries for now - e.g. save & save & add another fill the requirement no?

Reason for suggestion above is that normally people talk about symptom complex e.g Cough, fever, weight loss for 2 months Rather than distinct elements. Hope that sort of makes sense

Technical Plan:

• Add Role as a new model
• Add roles as new m2m field on UserProfile
• add default to userprofile.get_roles() roles['default'] = [r.name for r in roles.all()]
• Display a different modal interface depending on the value of e.g. profile.has_role('doctor')
• Have many checkboxes.
• Have a save all function in edit item
• DO SOMETHING APPROPRIATE

The current list contains a mix of vague and not vague options. I have discussed with Robin Bailey (consultant in charge) and he is happy that if the nurses are ok with it that we swap the vague symptoms ("Chest Symptoms") for more specific ones.

This will need discussion with nurses to check they are happy but I think we would go with something like:

• Fever
• Night Sweats
• Headache
• Myalgia
• Arthalgia
• Fatigue
• Weight Loss
• Sore throat
• Coryzal symptoms
• Rash
• Cough
• Shortness of Breath
• Nausea
• Diarrhoea
• Abdominal Pain
• Dysuria

It occurs to me the nurses will also need a Free-Text "Other" box

Just to say that nearly all of these are on the current tick sheet. We have replaced Chest symptoms > Shortness of Breath Abdominal Symptoms > Diarrhoea AND Abdominal Pain Urinary Symptoms > Dysuria

Discussed with Di (HTD Walk in nurse today) She seemed happy with entering this the same way as the doctors do - e.g text box. So no need for tick boxes. I think this makes this issue fixed.
