Details should include:

[x] Specimen type - from a lookup list to be supplied...
[ ] Date collected
[ ] Volumne
[x] Appearance - from a lookup list below
[ ] Epithelial cell
[ ] WBC (White blood cells)
[ ] Lab specimin number
[ ] Date shipped ( To research lab )
[ ] Biobanking y/n
[ ] Date biobanked (If Yes to ^)
[ ] Volume biobanked (If Yes to ^^)

Specimin types:

Broncho-alveolar lavage (BAL) induced sputum sputum, tracheal aspirate (TA), endotracheal secretions (ET secretions or ETT) Non-directed bronchoalveolar lavage (NBL), bronchial aspirates

Salivary Mucosalivary Mucoid Mucopurulent Purulent

All above can be "bloodstained"

Remove date shipped and replace by 'date tested'
