Reason for testing should have a "other" code.

In the logical model, Reason for testing is currently a codeableConcept. In the Questionnaire, it is an open choice. Both of these allow a selection from one of the predefined codes, or entry of a textual response. My understanding is that codes of "other" are discouraged, since adding a new value to the supported value set redefines what "other" can include. Therefore, current modelling is sufficient.
