Mostly implemented in questionnaire and logical model. Created issue #74 for enabling pregnancy trimester dependent on pregnancy comorbidity. Consistent with group consensus, did not add y/n/unk: a comorbidity is affirmatively asserted if present in the list, no assumption can be made about the comorbidity if absent from the list ("known, not applicable," "unknown if applicable," and "co-morbidity not in answer value set when asked" are some potential reasons for omitting). Although this may be limiting, it is believed other approaches have other shortcomings.
Switch to using codes for the condition and codes for whether it is present. (supporting y/n/unk)
