Should trimester be in the middle of the comorbidity entries? A sub-entry under pregnancy? A pregnancy value (pregnant? no, 1st trimester, 2nd trimester, ...)

With the refactoring of co-morbidities (#28), trimester is no longer mixed in with the list. This should be sufficient to resolve this issue. Note #74 addresses this issue in the questionnaire.
