Logical model "other" comorbidity should be a 0..* codeableconcept #30

Restructuring of comorbidities (#28) makes comorbidities into a 0..* list. A "other" value can show up multiple times. Note "other" as a coded value is left out per #24, but other values can be entered in the codeableconcept.text
