[RISC] Explore adding a reason_code claim #23

We currently have reason_admin and reason_user, there has been interest in exploring a standardized reason_code claim.

2022-08-23 call: wait for feedback, might not be feasible for v1

call 2023-03-21: agreed that this is nice to have, but don't hold up v1 spec
