UIowa requested the following:

[x] Make affiliations not required
[x] Make page numbers more flexible for chapters (allow range)
[ ] For an edited volume add the option to mark a contributor as an editor and display only the editors for edited vols

Non dev:

[ ] Load the oa logo
[x] Upgrade to latest plugin version
