Documentation: remove hard-coded references to section numbers #126

Example:

qcd ([section 7.7](#qcd-marker))|Properties from the quantization default (QCD) marker segment (codestream main header)|

These break if a new section is inserted somewhere, which is real PITA when updating the docs.
