Once a CBO/Org has signed up, but want to claim more records throughout any time

add on top menu "Claim Record"
same font, size, style as other menu options
Use this icon just as the referral icon is used, if the user claims it should show how many records they have claimed so far

see comment on #1323 - the top of the home page will already display their claimed records, so should we just continue to use that?

We could also add under 'have more data to contribute' with two buttons 'add a new record' and 'claim an existing record'

yes, will merge this one into #1323 with claim more records under 'have more data to contribute'
