Create a HTML mock-up of the new analysis request form.
This will be used by Brian to present to the external stakeholders. It will also be used internally to answer questions about what data we need to collect and what the user experience will look like.
The analysis request form will need to be extended to include:
A second codelist field
Relative time frames for each codelist (work with Louis to determine what these will be)
Provide option to search for a clinical events codelist or a medications codelist (for either codelist 1 or codelist 2)
Provide a radio button for events/medications, then retrieve the codelists from opencodelists
Allow the ability to just select one codelist, if required
Codelist selection could be from a detailed list rather than a select dropbox
Option to create their own codelist (this should be a link to Open Codelists for now)
The logical relationship between the two codelists (check with Louis what operators will be allowed)
Demographics breakdown options (checkboxes)
Sex
Age groups
Ethnicity
IMD
Population selection (adults/children/all) (dropdown?)
Option to breakdown by region (checkbox? Check with Louis)

A mock-up/wireframe that can be shared with Louis, Caroline and external users.
Screenshots that Brian is happy to show to external stakeholders.
Decision from Tom O about whether a multi-page wizard is required.
