Suggested Improvements to First Health Inequalities Report

This report is look great @LFISHER7 . The following is some small suggestions

• [x] Prominent contact details - team@opensafely.org - this should be up top somewhere. MIght be better handled in overall report framework though?
• [x] Link to underlying open code. Poss similar that it should be in the overall framework?
• [x] Linked codelists - they all link to richard-croker namespace which is the old location..... reminder for updating!
• [x] Zero axes
• [x] On side by side charts can we get the axes to match?
• [x] Medication review text. Added suggestion here. @richiecroker / @orlamac will review
• [x] Learning disability: instead of true / false would "record of a learning disability" / "no record of a learning disability" be better?
• [x] Caveats - where we have "none" shall we remove it from the report? This was to only to help writers of each measure.
• [x] Asthma - the last bullet point is missing review in the first sentence. Doesn't make sense otherwise!
• [x] Asthma/copd - the chart titles say rate of asthma/copd all of these need reviews
