Add remaining sentinel measures #7

This adds in the codelists for the remaining sentinel measures and adds them to the study definition and output notebook.

Let's have a chat sooner rather than later about how to tidy up this repo to make maintenance easier. At the moment, I fear the bus factor is tending to one.

Either way, though, I've checked out the branch and run opensafely run run_all and everything works 🙂
