Add clinical vulnerability variables to the "snapshot" dataset definition #7

PRIMIS is the company who took the "high risk" or "clinically vulnerable" groups defined by JCVI and turned them into clinical codes for identification in GP records.

These definitions have been updated over time, both by JCVI to include/exclude particular groups, and by PRIMIS to fix bugs in earlier versions of the specification.

The latest version that we have in opensafely (that I know about) is captured in the dataset definition in the comparative spring booster 2023 repo (via a PR led by Em Prestige). That dataset definition is the best place to start to get these variables added to the snapshot dataset definition.

For reference:

This folder in the OS drive contains all versions of the spec up to v2.5. This is the version that Em implemented.

Partially addressed by #9, with the structure now in place, but more variables to be added
