The two contributions of this PR are first, to provide a clear README.me and second, to have a dataset definition (ehrQL) rather than a study definition (cohort-extractor). It isn't clear why the dataset definition/study definition needs to be this involved (surely one call to codelist_from_csv would suffice?), but we can address that in the future.
Because this is a seldom visited repository, I've bundled up both contributions into a single "Stoppit and Tidyup" PR.
