Hey so I just did a really basic skeleton with some validation and address auto complete for a user to enter their information. I wasn't really sure how we wanted to handle the UI/styling so I left it very bare bones for now. @ttglennhall I saw that you wanted to start implementing designs in Gitter so I thought I would maybe pull request this for now so you could have something to work off of and then I would start on the submitting/sending the data and catching it on the back-end tomorrow? Let me know if you want me to change anything or I should work on some thing else.
