[BUG] UX changes required for page header changes

What is the bug? There are a few issues with the page header implementation

• Duplicate role/edit user not showing username in breadcrumbs
• Modal options for create permission not full width
• Possibly extra space on audit log tab

How can one reproduce the bug? Visit each of the specified above pages with the new home page experience feature flag on. See screenshots below
