Adding Issue Tracking Document and Template #60

issue_tracking.md details the basic flow of tracking an issue.

Note: this flow is the base issue flow; there could be other flows (such as needing more info on the issue, re-evaluating the issue, then approving the issue).

ISSUE_TEMPLATE.md will be added to each repository upon approval of this pull request.

Notice the labels added in each repository. The document repos contain slightly different labels, e.g., document repos will not have defect/product code labels but will have addition/document repos.

This issue addresses part of #51 by creating the template.
