This PR deprecates orphan mitigation and adds a failure cleanup section to the Provisioning and Binding sections.

Closes #397, closes #370, closes #369

Travis is printing a couple of cacophonic errors

line 856: 'Should' should be 'SHOULD' line 1273: 'Should' should be 'SHOULD'
