Optimize for mobile #53

In #47 @RohitTalwalkar mentioned the landing page and form isn't very mobile friendly (though the results page is). Would it be possible to optimize the entire site for mobile?

Many, if not a majority, of users will visit the site from mobile devices.

@saadmir I wanted to check to see if this was pushed to http://getfoodsanmateocounty.org. Looks great on wic.surge.sh!
