Duplicate data should not be allowed on Name Filed in all other module #25

Duplicate data should not be allowed on Name Filed in Cohort type, Add Cohort Attributes Type, Add Cohort Member Attribute Type, Add Cohort Programs and Add Cohort Role (High).

Incase I am not able to resolve this for all metdata, I have fixed this for the main 'Add Cohort' and 'Add Cohort Members'. That means that from now on cohorts with duplicate names are not allowed. Duplicate patients cannot be added to the module

@maimoonak this is alright yes? The duplication condition?

It's working fine on other module but still allowed duplicate data on Manage cohort attributes type and Manage Cohort Member Role

Can you plz add a tag 'reopened' on all reopened issues. I will try to resolve them
