• [x] Let's change the name of this form to "Close ANC Record" CG: Please change
• [x] Don't capitalize "E" in Wrong entry
• [x] Don't include the years in parentheses when entering date of death or date of miscarriage/abortion or date of delivery.
• [x] Why are the date questions formatted differently from the other questions? They seem to be greyed out and have a smaller left margin than the others.
• [x] Cause of death should be a required field CG: still no asterisk
• [x] In the cause of death reasons, the antepartum hemorrhage answer choice needs to copy to the next line, right now it gets cut off the page.
• [x] We had "abruption" spelled wrong in the antepartum hemorrhage answer choice. Sorry about that. It is fixed now. Should be "abruption", not "abrumption".
• [x] Pls change to Postpartum haemorrhage (no hyphen)
• [x] Use "Delivery date" instead of the description for that field. CG: Don't capitalize "date"
• [x] Vacuum is mispelled.
• [x] Birth weight should not be required. CG: Asterisk still on here
• [x] Constraint logic missing on birthweight field (> 0 and <= 10)
• [x] Any delivery complications is formatted very differently from the rest - larger font, etc. Is it possible to make all the fields aligned, same font, text size, etc?
• [x] In delivery complications answer choices, sorry the spelling wasn't up to date. Pls change previa to "praevia". We have to use the British spelling throughout the app.

• [x] Change to (2nd, 3rd, or 4th degree) CG: remove extra spaces before "4th"
• [x] Pls add data constraint logic to delivery complications field (Cannot select "None" with any other option)

• [x] Please revert the exit form confirmation message to what is was before. The "changes" message is for forms that were opened for editing, not new forms opened like this one. Also, there is no reason to put "changes" in quotations.

• [x] This form is broken. Please check all the skip logic. When I open the form, instead of seeing the first question, I'm seeing the specific fields for the live birth option only.
