What kind of support do you need?

In the Tests Page, there is the Ultrasound Test section, after the number of fetuses; add the following field:

Trimester 1
Trimester 3

When one selects Trimester 1, add the following fields:

[ ] GS (Gestational Sac)
[ ] CRL (Crown-rump Length)
[ ] FHR (Fetal heart rate)
[ ] Location of the Gestational Sac
[ ] Intrauterine
[ ] Extra-uterine
[ ] Expected date of Delivery - Date

When one selects Trimester 3, show the following fields:

[ ] Fetus
[ ] Alive
[ ] Dead
[ ] Fetal location
[ ] Intrauterine
[ ] Extra-uterine
[ ]Fetal Presentation
[ ] Cephalic
[ ] Pelvic
[ ] Transverse
[ ] Other - field
[ ] Fetal weight (gram)
[ ] Placenta location
[ ] Praevia
[ ] Low
[ ] Anterior
[ ] Posterior
[ ] Right side
[ ] Left side
[ ] Fundal
[ ] BPD (cm) - integer (2 digits)
[ ] HC (cm) - integer (2 digits)
[ ] AC (cm) - integer (2 digits)
[ ] FL (cm) - integer (2 digits)
[ ] SPD amnion(cm) - integer (2 digits)
[ ] Gestational Age (week) - integer (2 digits)
