Fixed medical history showing on ANC/PNC/Child/Malaria and FP Member profiles

[x] Added missing migration to add a column to the visits table
- The missing column led to an Exception when retrieving visits.

[x] Downgraded Guava to v20. v24+ breaks forms in Android < 7
- This is to fix Forms crashing the app in Android 5
