• [x] Native form widget split into 3 screens: Stop current method (row 4 -11), Family planning method (row 14 - 16), Family planning method info (row 18 - 25)
• [x] When {reason_stop_fp_chw} != "Decided to change method" the record is archived, the man is removed from the family planning register, and the family planning row is removed from his profile
• [x] When {reason_stop_fp_chw} = "Decided to change method", the man's record is updated with the new family planning method chosen, new schedules are calculated, the family planning row is updated
