Forms available to clients based on their profile #65

The forms available to a client is based on their profile:

All clients, where {wra}=0 and {age}>5 years, and is not in a register: Malaria confirmation

@cafootitt @rowo Do we want to include a "Other" referral or information if referral reason doesn't exist? (Logic being there are many reasons to refer, so explanation could say the app doesn't cover whatever other reason there is so it doesn't need to be recorded. It would also prevent the "which referral" screen from ever only having one option) No action would be taken on the HF, so I am not so sure how helpful this would be. What are your thoughts? It might also be out of scope.

@msschroeder There are probably things the CHW does that aren't covered in the app beyond other referral reasons, so I'm thinking this is something the client should bring up and request. However if it's not something that impacts amount of our work, then you could bring it up to them (since it makes no difference to us). I feel like anything we'd come up with without being on the ground implementers would be anticipating potential problems that may or may not be applicable to this scenario.
