Change the label of the following questions on Remove Member form;

• [x] Does the family of the decedent have a death certificate (Y/N)? --> Does the family of the decedent have a death certificate?
• [x] Death certificate issuance date (if certificate received) --> Death certificate issuance date
• [x] Death certificate number (if certificate received) --> Death certificate number
• [x] Was the death notification done (if certificate not received) - Y/N)? --> Was the death notification done (if certificate not received)?
• [x] Implement relevance of the above questions as outlined on DD above
