CRVS: Intrapartum (fresh) stillbirth deaths appearing as death of mother in death certification list

Using 4Feb2022 release. I recorded a Pregnancy outcome as stillbirth. The woman is showing in the death certification list (and still appearing in the household list).

Steps to replicate:

1. Enter Pregnancy outcome form
2. Select stillbirth
3. Enter delivery date of yesterday
4. Select health facility
5. Select intrapartum stillbirth
6. Select nurse
7. Select unknown
8. Select singleton
9. Save form
10. Check Death certification list. Mother appears as deceased without specifying "[Stillbirth]"

Note: if you select "antepartum stillbirth", the death is correctly listed as "CG: mother's name, [Stillbirth] Patient Name"

This was either never a bug or resolved in the 14Feb release. Ager was not able to reproduce this. The woman could have appeared in the death certification list because I removed her from the family previously while testing issue #2000. However, I never registered her death. New issue #2002 created to reflect that.
