CRVS: Error with mother's age in out of area birth form

When registering a new Out of area birth, there are a few issues:

• Child's age is automatically calculated and displayed in line with the date of birth. This is fine, but please remove "Child's age in days, months, or years" which appears to be hard-coded in the form.
• Mother's age is automatically calculated and displayed in line with the date of birth. However, "age" is appearing as a separate field and mandatory. It should be automatically calculated if DOB is known / not a text entry field.
• The age field should only appear if "unknown" for DOB is checked. See relevance and constraints in the data dictionary.
• The constraint for the mother's age is incorrect. It is saying "Age must be equal or less than 11." It should be between 5 and 120.
