CRVS: Add parent details on U5 Registration form

From UAT feedback, there's need to add parent details to children under 5 to facilitate their birth registration process. The details are similar to those captured under child registration for out of area. The U5 registration form has been updated to reflect the additional parent details as indicated in the data dictionary under add child under N They include;

[x] Name of birth place
[x] Number of children born
[x] Type of attendance at delivery
[x] Mode of delivery
[x] Type of pregnancy
[x] Baby weight
[x] Mother's name
[x] Mother's national ID
[x] Mother's DOB
[x] Mother's Marital Status
[x] Mother's Education Level
[x] Mother's Residence
[x] Mother's Birth Place
[x] Father's name
[x] Father's national ID
[x] Father's DOB
[x] Father's Marital Status
[x] Father's Education Level
[x] Father's Residence
[x] Father's Birth Place
