Add all facilities location hierachy for referrals #469

On all referrals on the OPD form; Vital danger signs check, Lab, pharmacy, final outcome, list should have names of health facilities not village names.

@AnnieMungai Please update the data-dictionary for the issue

@hamza-vd I do not think this issue requires data dictionary updates.

Any place on OPD that has a location hierarchy should be pulling all facilities under Ntcheu-DHO and not villages.

Note! Make sure the implementation does not affect any other part of the app.

@AnnieMungai Yes, that's noted. I misunderstood it earlier.

Location levels are not shown on the server side. It is not clear how the location hierarchy looks like.
