Cannot register client from the PNC register

v.0.4.7-preview When you try to register a client from the PNC register, the NEXT button does not work and user is not able to fill all details and save.

Acceptance criteria

• [x] When user taps on the + button, the PNC registration form opens up.
• [x] User can tap on the NEXT button on the form and complete filling all the details.
• [x] Once all the details are filled in, user can save the form and the PNC client is added to the register.
• [x] The registered patient can be seen on the PNC register and the All clients register.

@AnnieMungai I can confirm that this is now working.

This has been fixed here on GIZ and here on native form

This will be released together with the other bugs on the next APK

@ekigamba this seems to affect other forms too - ANC and maternity outcome. Will this fix resolve for all of them?

QA Findings:

1. I was able to click on PNC Clients on the hamburger menu
2. Click register and fill in the details of a patient
3. Click next and submit.
4. I will test with other forms such as ANC and maternity outcome.
5. At the moment, this has passed QA
