Create child/caregiver registration form #9

[x] Update the registration form. XLSForm version of the Registration form can be found via this link. The JSON version of the form (may need updating) can be found here
[x] Title of form: Registration (changed from Birth Registration on image below to Registration)
[x] It will use a style similar to below.
[ ] Enable multiple languages: English/Portuguese

QA release October 29th:

[x] Can we update the constraint for the child's age to be less than or equal to 2 months. I have updated the registration form and highlighted the row in yellow. I added the constraint and constraint message
[ ] Can we provide the age for the child, similar to the caregiver Child age

Caregiver age

[x] Can we not make the second phone number required, BUT if a phone number is filled the following question must be answered
[x] Can we update the section headings to match a color scheme similar to what is there:
Reduce slightly by maybe 2 font sizes
Make the font color white (#FFFFFF)
Make the color of the background behind the text similar to the above color (#148bc7) @rowo What do you think? This is what it currently looks like

@msschroeder @hamza-vd Is there a place where all of the exiting form styles can be viewed? Maybe a sample form in an app or documentation?

Also, I'm having trouble following what the recommendation will change from and to. Can you throw it into a Google slide? Also, what are the constraints around updates to styling? I don't think it's as easy as changing a web form's CSS.

@rowo Here is an example of what it looks like in the app. Please let me know if you don't think this looks okay.
