"if an older child say 6 weeks comes in first contact with facility; assuming that the nurse will give opv1 and leave out OPV0 because it has passed 13 days ,the effect this has On the home page, is that vaccines status will still be showing ''record at birth'' even when the child is due for other vaccines. Perhaps an option to say OPV 0 was not given is needed."

I would like to suggest having these fields editable at-least within the day and can be locked out."
Recording a child who is 6 months activates Vitamin A IFC2 which is a dose for period 0 - 5 months. The system should activate Vitamin A 1 since its 6 months (However posses a challenge on back entry i.e it will mean that Vitamin A IFC2 cannot be back entered if it was given earlier. (This might need discussion.)
